Care Centers of Nassau is a not for profit service provider licensed as an Intermediate Care Facility for Developmentally Disabled individuals as well as an HCBS Waiver service provider.  Care Centers of Nassau provides for the physical, emotional, and personal needs of the individuals we serve. Our efforts and programs are based on comprehensive functional assessments.

Care Centers of Nassau operates three Intermediate Care Communities for the developmentally disabled each consisting of three eight-bedroom homes, with a total licensed capacity of 72 beds. In addition to the ICF/DD communities, CCN also operates one Independent Achievement Adult Day Program location in Fernandina Beach FL.
